Expert*innen aus der ganzen Welt beraten methodische Fortschritte in der Extraktion von Herzschrittmacher- und Defibrillator-Sonden
Bereits zum 10. Mal hat sich die internationale Community von Expert*innen der Schrittmacherextraktion zum „International Lead Extraction Expert Meeting“ (ILEEM) in Berlin versammelt.
Die Implantation von Herzschrittmacher- und Defibillator-Systemen bei Herzrhythmusstörungen ist heutzutage weit verbreitet und gewinnt an Bedeutung. Unter Umständen kann es bei einer Infektion des Systems, technischen Fehlfunktionen oder sondenbedingten Gefäßverschlüssen erforderlich sein, die Sonden wieder zu entfernen.
Die Entfernung des Schrittmacher- bzw. Defibrillator-Aggregats selbst ist in der Regel unproblematisch, da es im Unterhautfettgewebe oder unter dem Brustmuskel platziert und gut zugänglich sind. Die schlauch- bzw. drahtförmigen Sonden eines solchen Systems, die über das Venensystem in die Herzkammern gelegt werden, können jedoch mit den Gefäßwänden oder dem Herzmuskel selbst verwachsen. Die Entfernung der Sonden ist je nach Ausprägung der Verwachsungen entsprechend komplex.
Vor diesem Hintergrund haben sich renommierte Medizinerinnen und Mediziner aus allen Teilen der Welt im Dezember 2023 zu einem wissenschaftlichen Symposium unter Leitung von Prof. Dr. Christoph Starck in Berlin zusammengefunden, um sich zu neuen wissenschaftlichen Erkenntnissen und innovativen Techniken der transvenösen Sondenextraktion fortzubilden und anhand realer Fallbeispiele Erfahrungen auszutauschen.

10th International Symposium on Pacemaker Extraction: Experts from around the world discuss methodological advances in the extraction of pacemaker and defibrillator probes
For the 10th time, the international community of lead extraction experts gathered in Berlin for the “International Lead Extraction Expert Meeting” (ILEEM).
The implantation of pacemaker and defibrillator systems for cardiac arrhythmia is widespread today and of increasing importance. Under certain circumstances, it may be necessary to remove the probes in the event of an infection in the system, technical malfunctions or a vascular occlusion caused by the probe.
The removal of the pacemaker or defibrillator unit itself is generally unproblematic, as it is placed in the subcutaneous fatty tissue or under the chest muscle and is easily accessible. However, the tubular or wire-shaped probes of such a system, which are placed in the heart chambers via the venous system, can grow together with the vessel walls or the heart muscle itself. Depending on the extent of the adhesions, the removal of the probes is correspondingly complex.
